Prospective Students

Items to be submitted with application:

• Two forms of identification must be provided. 

​

One form of identification must be a U.S. government issued Social Security card, signed and non-laminated.

​

          The second form of identification must be one of the following:

             - Current, non-expired license 

             - U. S government-issued Military ID (if not used in lieu of social security card)

             - State-issued identification card

             - Passport (US or foreign, current, non-expired)

             - Current, non-expired, federal-issued employment authorization document (EAD) photo                       id card.

             - Alien registration card

             - Current, non-expired NC Learners Permit.

• Current CPR certification

• Immunization records

• TB skin test

• High school transcript or GED transcript

• Criminal background check by designated vendor (once accepted in program)

• Drug Test by designated vendor (once accepted in program)
